Study summary

Objective: To quantify and compare total staff person-time per imaging episode betweenconventional chest radiography (CXR) and ULDCT in adult emergency department patients in whom CAP is a differential diagnosis.

Read the detailed description

Background: Community-acquired pneumonia (CAP) is a leading cause of acute hospital admission. Chest radiography, the standard first-line imaging test, has limited sensitivity, whereas ultra-low-dose CT (ULDCT) offers higher diagnostic accuracy at a comparable radiation dose. The staff person-time implications of substituting ULDCT for radiography in emergency department workflow are unknown.

Objective: To quantify and compare total staff person-time per imaging episode between conventional chest radiography (CXR) and ULDCT in adult emergency department patients in whom CAP is a differential diagnosis.

Methods: Single-centre, two-period observational time-and-motion study at the emergency department of Southern Jutland Hospital - Aabenraa. The CXR arm was collected from 2025 to June 2027; the ULDCT arm is collected from September 2026. The primary outcome is cumulative staff person-minutes per imaging episode, combining electronic radiology-system timestamps with stopwatch-measured in-room occupancy time. The primary analysis is a Mann-Whitney U test with a bootstrapped confidence interval for the difference in medians, supplemented by adjusted quantile regression.

Inclusion criteria

  • Patients presenting to the emergency department during daytime weekday hours in whom CAP is a differential diagnosis, and who are able to undergo the assigned imaging examination.

Exclusion criteria

Exclusion Criteria:

  • Patients requiring immediate imaging for a critical condition (for example suspected pneumothorax).
  • Bariatric patients exceeding the technical limits of the scanner
  • Pregnancy
  • Patients requiring additional imaging beyond the chest (for example abdominal CT)
  • Severe dementia, or need for sedation to complete imaging
  • Patients requiring mechanical ventilation.
  • Patients requiring accompaniment by medical staff during transport

What researchers measure

Primary outcomes

  1. Cumulative staff person-minutes per imaging episode

Total human resource utilisation, defined as cumulative staff person-minutes per imaging episode, from the point staff fetch the patient for imaging to the point a final radiological report is available. The time contributed by every involved staff member is summed (multi-staff summation rule: two staff members working simultaneously for two minutes contribute four person-minutes). Contributing personnel categories: porters, radiographers, nurses, and radiologistsor reporting radiographers.

Time frame: From admission to completon of imaging, up to 1 week.

Secondary outcomes

  1. Portering/transport time

From staff fetching the patient at the ward room to arrival at the imaging room measured in minutes

Time frame: From admission to arrival at imaging, up to 24 hours.

  2. in-room occupancy time

When the patient arrives at imaging until patient leaves imagining department

Time frame: From arrival at imaging to discharge from imaging, up to 24 hours

  3. image acquisition time

Time (minutes) taken to complete the imaging

Time frame: From time imaging begins to time imagining ends, up to 60 minutes

  4. reporting time

The time (minutes) taken according to the electronic registration system

Time frame: From imaging completion to completion in electonic registration system, up to 60 minutes

What these dates mean
Start.
When the first participant enrolled; while estimated, when the study expects to open to recruitment.
Primary completion.
When the last participant was examined or treated to collect data for the study's main question, its primary outcome. A study that stopped early gives the date its data collection stopped.
Completion.
When the last participant's last visit took place, for every outcome, secondary ones and side effects included: the end of the study. Often later than primary completion, sometimes the same day.
Actual and Est.
Actual means the registry says the date has happened. Est. (estimated) is a date the sponsor plans or expects. It can still move, and the sponsor is expected to change it to actual once it has passed.
