This is a prospective, single-arm, multicenter, phase II clinical study designed to evaluate the efficacy and safety of iparomlimab and tuvonralimab (QL1706) combined with cetuximab and mFOLFIRI as first-line treatment for recurrent or metastatic colorectal cancer. A total of 55 participants with low-level microsatellite instability (MSI-L) or microsatellite stable (MSS)/mismatch repair-proficient (pMMR) and RAS/BRAF wild-type tumors will be enrolled. Enrolled participants will receive iparomlimab and tuvonralimab (5 mg/kg, Q3W, Day 1) + cetuximab (500 mg/m², Q2W, Day 1) + irinotecan (180 mg/m², Q2W, Day 1) + leucovorin (LV) (400 mg/m², Q2W, Day 1) + 5-FU (400 mg/m², Q2W, Day 1). After 12 cycles of the chemotherapy phase, participants who achieve CR, PR, or SD will then receive maintenance therapy with iparomlimab and tuvonralimab (5 mg/kg, Q3W, Day 1) + cetuximab (500 mg/m², Q2W, Day 1) + leucovorin (LV) (400 mg/m², Q2W, Day 1) + 5-fluorouracil (5-FU) (400 mg/m², Q2W, Day 1) until disease progression, intolerable toxicity, investigator determination that the participant is no longer benefiting, withdrawal of informed consent, completion of 2 years of iparomlimab and tuvonralimab treatment, or other protocol-specified reasons.

Drug: QL1706 combined with cetuximab and mFOLFIRI
QL1706 (5 mg/kg, Q3W, Day 1) + cetuximab (500 mg/m², Q2W, Day 1) + irinotecan (180 mg/m², Q2W, Day 1) + leucovorin (LV) (400 mg/m², Q2W, Day 1) + 5-FU (400 mg/m², Q2W, Day 1). Progression-Free Survival (PFS)
Progression-free survival (PFS) is defined as the time from enrollment/randomization/first dose to the first documented disease progression (per RECIST v1.1) or death from any cause, whichever occurs first. Participants without a PFS event will be censored at the date of the last tumor assessment.
Time frame: From first dose until documented disease progression (per RECIST v1.1) or death from any cause, whichever occurs first (up to approximately 24 months)
Objective Response Rate (ORR) Assessed by Investigators per RECIST v1.1
Objective response rate (ORR) is defined as the proportion of participants with a best overall response of complete response (CR) or partial response (PR) as assessed by the investigator according to RECIST v1.1.
Time frame: From first dose until documented disease progression, initiation of new anticancer therapy, death, or data cutoff, whichever occurs first (up to approximately 24 months).
Disease Control Rate (DCR) Assessed by Investigators per RECIST v1.1
Disease control rate (DCR) is defined as the proportion of participants with a best overall response of complete response (CR), partial response (PR), or stable disease (SD) as assessed by the investigator according to RECIST v1.1.
Time frame: From first dose until documented disease progression, initiation of new anticancer therapy, death, or data cutoff, whichever occurs first (up to approximately 24 months).
Duration of Response (DoR) Assessed by Investigators per RECIST v1.1
Duration of response (DoR) was defined as the time from the first documentation of objective response (complete response \[CR\] or partial response \[PR\]) to the first documentation of disease progression per RECIST v1.1 or death from any cause, whichever occurred first, in participants who achieved a confirmed CR or PR as assessed by the investigator. Participants without an event were censored at the date of the last tumor assessment.
Time frame: From first documentation of objective response (CR or PR) to documented disease progression per RECIST v1.1, death from any cause, or data cutoff, whichever occurs first (up to approximately 24 months).
12-Month and 24-Month Overall Survival Rates
The proportion of participants who were alive at 12 months and 24 months after the first dose, estimated using the Kaplan-Meier method. Participants who had not died or were lost to follow-up at the data cutoff were censored at the date last known alive.
Time frame: 12 and 24 months after first dose
Overall Survival (OS)
Overall survival (OS) is defined as the time from first dose (or enrollment/randomization, as specified in the protocol) to death from any cause. Participants who are alive or lost to follow-up at the data cutoff will be censored at the date last known alive.
Time frame: From first dose to death from any cause or data cutoff (up to approximately 24 months).
