Study summary

The goal of this exploratory study is to evaluate the efficacy and safety of zeprumetostat in combination with retlirafusp alfa, apatinib, and chemotherapy as first-line treatment in patients with HER2-negative unresectable locally advanced or metastatic gastric/gastroesophageal junction adenocarcinoma who have not received prior anticancer therapy.

Read the detailed description

Participants will receive zeprumetostat combined with retlirafusp alfa, apatinib, and the CAPOX regimen (Q3W) at the RP2D determined from the dose-escalation phase. Tumor response evaluation will be performed every 2 treatment cycles (data analysis includes the 6 patients from the dose-escalation phase).

  16. Pregnant (positive pregnancy test before medication) or lactating women;
  17. Any other diseases, clinically significant metabolic abnormalities, physical examination abnormalities or laboratory test abnormalities, as judged by the investigator, suggesting that the patient has a certain disease or condition that is not suitable for using the study drug (such as having epilepsy and requiring treatment), or will affect the interpretation of the study results, or place the patient at a high risk;
  18. Urinalysis indicates that urine protein is ≥ 2+, and the 24-hour urine protein volume is > 1.0g;

  20. Participating in another interventional clinical study at the same time, unless participating in an observational (non-interventional) clinical study or in the follow-up stage of an interventional study; patients considered not suitable for inclusion in this study by the researcher.

What researchers measure

Primary outcomes

  1. Objective Response Rate (ORR)

    Objective Response Rate (ORR) is defined as the proportion of participants achieving a confirmed complete response (CR) or partial response (PR) according to RECIST v1.1 criteria.

    Time frame: From first dose to disease progression or death (assessed up to 2 years)

Secondary outcomes

  1. Progression free survival (PFS)

    Progression-Free Survival (PFS) is defined as the time from the date of first dose to the first documented disease progression (according to RECIST v1.1) or death due to any cause, whichever occurs first.

    Time frame: From first dose to disease progression or death (assessed up to 2 years)

  2. Overall survival (OS)

    Overall Survival (OS) is defined as the time from the date of first dose to the date of death due to any cause. Participants who are alive at the time of analysis will be censored at the last known alive date.

    Time frame: From first dose to death due to any cause (up to 5 years)

  3. Disease Control Rate (DCR)

    Disease Control Rate (DCR) is defined as the proportion of participants who achieve a confirmed complete response (CR), partial response (PR), or stable disease (SD) according to RECIST v1.1 criteria.

    Time frame: From first dose to disease progression or death (assessed up to 2 years)

  4. During of Response (DOR)

    Duration of Response (DOR) is defined as the time from the first documentation of objective response (CR or PR) to the first documentation of disease progression or death due to any cause, whichever occurs first. This will be evaluated only in participants who achieve an objective response.

    Time frame: From first documentation of response to disease progression or death (assessed up to 2 years)

  5. Incidence of Treatment-Emergent Adverse Events (TEAEs)

    Incidence, severity, and relationship to study drug of treatment-emergent adverse events (TEAEs) and serious adverse events (SAEs) according to NCI CTCAE v6.0.

    Time frame: From first dose to 90 days after the last dose (up to 2 years)
