A Phase 2 interventional study of Becotatug Vedotin (MRG003) and Pucotenlimab in Metastatic Colorectal Cancer, sponsored by Fudan University. Recruiting at 1 site in China. Open to participants aged 18 Years and older. Per ClinicalTrials.gov, last updated 2026-10-20.
Sponsored by Fudan University · Phase 2, Interventional, and Treatment
The BECOME trial will study whether becotatug vedotin combined with pucotenlimab can shrink tumors and how safe this treatment is in adults with colorectal cancer that has returned or spread after at least two prior lines of standard treatment. The study described in the informed consent form is for patients whose tumors test positive for a protein called epidermal growth factor receptor (EGFR).
Becotatug vedotin, also known as MRG003, is an antibody-drug conjugate (ADC). It combines an antibody that recognizes EGFR with a drug designed to kill cancer cells. Pucotenlimab is an immunotherapy that blocks a protein called PD-1 and may help the immune system recognize and attack cancer cells. The benefit and risks of using these two medicines together in this patient group are being investigated.
This study plans to enroll 31 participants at Fudan University Shanghai Cancer Center in China. All participants will receive the same combination treatment. There is no comparison group, and both participants and study doctors will know which medicines are being given. Participants must be at least 18 years old, have at least one tumor that can be measured on a scan, and have adequate organ function and physical fitness for treatment. Doctors will review medical history, examine participants, and perform tests to check eligibility before treatment begins.
Participants will receive both medicines through an intravenous infusion once every three weeks. Pucotenlimab will be given at 200 mg, and becotatug vedotin at 2.0 mg per kilogram of body weight. Treatment may continue for up to two years but may stop earlier if the cancer worsens, side effects become unacceptable, a participant withdraws consent, or another reason specified in the study plan occurs. Study doctors may pause treatment, adjust the dose where appropriate, or stop treatment for safety reasons.
Participants will attend regular visits for examinations, blood and urine tests, heart checks, and monitoring of side effects. Tumor scans will be performed every two treatment cycles during the first six cycles and every three months afterward. A safety follow-up will take place about 30 days after the last dose, followed by survival follow-up approximately every 12 weeks.
The main outcome is the proportion of participants whose tumors shrink or disappear on scans. Researchers will also assess how long responses last, how long participants live without their cancer worsening, overall survival, and side effects. Possible risks include low blood cell counts, skin reactions, liver test abnormalities, nerve problems, and infusion reactions. Other serious or unexpected side effects may occur. Benefit is not guaranteed. Participation is voluntary, and participants may withdraw without affecting their usual medical care.

Exclusion Criteria:
Participants will receive pucotenlimab 200 mg by intravenous infusion, followed by becotatug vedotin 2.0 mg/kg by intravenous infusion, on Day 1 of each 21-day cycle. Treatment will continue for up to 2 years or until disease progression, unacceptable toxicity, withdrawal of consent, initiation of another anticancer treatment, or another protocol-defined reason for discontinuation, whichever occurs first.
Drug: Becotatug Vedotin (MRG003) · Drug: Pucotenlimab
Becotatug vedotin is an EGFR-targeted antibody-drug conjugate. It will be administered at 2.0 mg/kg by intravenous infusion on Day 1 of each 21-day cycle, after the pucotenlimab infusion. Treatment will continue for up to 2 years or until a protocol-defined discontinuation criterion is met.
Pucotenlimab is a humanized monoclonal antibody targeting PD-1. It will be administered at a fixed dose of 200 mg by intravenous infusion on Day 1 of each 21-day cycle, before the becotatug vedotin infusion. Treatment will continue for up to 2 years or until a protocol-defined discontinuation criterion is met.
Objective Response Rate (ORR)
Percentage of participants whose best overall response is complete response (CR) or partial response (PR), assessed according to Response Evaluation Criteria in Solid Tumors (RECIST)
Time frame: From the first dose of study treatment through the end of tumor response assessment, up to 24 months.
Disease Control Rate (DCR)
Percentage of participants whose best overall response is confirmed complete response (CR), confirmed partial response (PR), or stable disease (SD), assessed according to RECIST version 1.1.
Time frame: From the first dose of study treatment through the end of tumor response assessment, up to 24 months.
Duration of Response (DOR)
Time from the first documentation of complete response (CR) or partial response (PR) to disease progression according to RECIST version 1.1 or death from any cause, whichever occurs first. DOR will be assessed in participants with a confirmed objective response. Participants without progression or death will be censored at their last tumor assessment.
Time frame: From the first dose of study treatment through the end of tumor response assessment, up to 24 months.
Progression-Free Survival (PFS)
Time from the first dose of study treatment to disease progression according to RECIST version 1.1 or death from any cause, whichever occurs first. Participants without progression or death will be censored at their last tumor assessment.
Time frame: From the first dose of study treatment to disease progression or death, assessed up to 24 months.
Overall Survival (OS)
Time from the first dose of study treatment to death from any cause. Participants without a reported death at the time of analysis will be censored at the date they were last known to be alive.
Time frame: From the first dose of study treatment to death from any cause, assessed up to 24 months.
Incidence and Severity of Treatment-Emergent Adverse Events (TEAEs)
Percentage of participants experiencing treatment-emergent adverse events. Events will be summarized by type and severity, with severity graded according to the National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE) version 5.0.
Time frame: From the first dose of study treatment through 30 days after the last dose.
Incidence and Severity of Treatment-Related Adverse Events (TRAEs)
Percentage of participants experiencing adverse events considered related to either study drug by the investigator. Events will be graded according to NCI-CTCAE version 5.0. The percentage of participants experiencing Grade 3 or higher treatment-related adverse events will also be reported.
Time frame: From the first dose of study treatment through 30 days after the last dose.
Incidence of Serious Adverse Events (SAEs)
Percentage of participants experiencing serious adverse events. Events will be summarized by type, severity, and relationship to study treatment. Seriousness and severity will be assessed separately, with severity graded according to NCI-CTCAE version 5.0.
Time frame: From the first dose of study treatment through 30 days after the last dose, with subsequent collection of study-treatment-related SAEs as specified in the protocol.
Start is estimated for Oct 2026; primary completion for Oct 2028, 24 months later.
Of 476 non-industry phase 2 interventional studies under Colorectal Neoplasms, half reached primary completion within 50 months of starting (the median), counting those still running; a quarter within 32 and three quarters within 72. This study's planned 24 months is below that.
Counting every sponsor, half of 584 studies reached it within 47 months; a quarter within 30 and three quarters within 72.
