Study summary

This randomized platform trial compared ciprofol and propofol for early neurocognitive recovery in adults undergoing painless gastrointestinal endoscopy. The primary outcome was the incidence of delayed neurocognitive recovery (dNCR) at PACU discharge, assessed via MoCA, DST, SMDT, STT, and Stroop tests (≥ 1 SD decline from baseline in ≥ 2 domains). Secondary outcomes included PACU stay duration, total drug dosage, and adverse events (hypoxemia, hypotension, bradycardia, body movement). Assessments were performed at baseline and before PACU discharge (Modified Aldrete ≥ 9).

Study population

Adult outpatients aged ≥ 18 years who were scheduled for elective painless gastrointestinal endoscopy and met the predefined inclusion and exclusion criteria were enrolled. Key inclusion requirements were ASA physical status I-III with stable comorbidities and provision of signed informed consent. Patients with psychiatric disorders, cognitive impairment, substance or alcohol abuse, BMI > 30 kg/m², severe organ dysfunction, or known hypersensitivity to ciprofol, propofol, or opioids were excluded.

Go through the criteria one by one — yes, no or not sure — and take the list to your doctor or the study team. Kept in this browser only.

Inclusion criteria

  • ASA physical status I, II, or III (concurrent diseases stable for ≥ 3 months with documented optimization)
  • Scheduled for elective painless gastrointestinal endoscopy under sedation
  • Provision of signed informed consent by the patient or legally authorized representative

Exclusion criteria

Exclusion Criteria:

  • Known psychiatric disorders, cognitive impairment, or inability to cooperate with preoperative evaluation
  • Confirmed or suspected history of abuse or dependence on narcotic, sedative, or analgesic drugs
  • Long-term alcohol abuse
  • Body mass index (BMI) > 30 kg/m²
  • Severe cardiac, pulmonary, hepatic, or renal disease
  • Known hypersensitivity to ciprofol, propofol, opioids, or related agents

What researchers measure

Primary outcomes

  1. Incidence of delayed neurocognitive recovery (dNCR) at PACU discharge

Assessed at baseline (pre-procedure) and before PACU discharge (Modified Aldrete score ≥9). Tests: MoCA, DST, SMDT, STT, and Stroop test. dNCR defined as ≥1 SD decline from baseline in ≥2 cognitive domains.

Time frame: Baseline (pre-procedure) and before PACU discharge (Modified Aldrete score ≥9)

Secondary outcomes

  1. PACU stay duration

time from PACU admission to discharge (Modified Aldrete score ≥9).

Time frame: Intraoperatively to PACU discharge

  2. total anesthetic dosage

cumulative ciprofol or propofol used intraoperatively (mg and mg/kg).

Time frame: Intraoperatively to PACU discharge

Other outcomes

  1. Incidence of hypoxemia

Defined as SpO2 \<90% during procedure or PACU stay, requiring intervention (e.g., airway support, oxygen).

Time frame: Intraoperatively to PACU discharge.

  2. Incidence of hypotension

Defined as SBP \<80% of baseline or requiring vasoactive intervention during procedure or PACU stay.

Time frame: Intraoperatively to PACU discharge.

  3. Incidence of bradycardia

Defined as heart rate \<50 beats/min or requiring intervention during procedure or PACU stay.

Time frame: Intraoperatively to PACU discharge.

  4. incidence of body movement

Defined as intraoperative body movement affecting endoscopic operation, requiring additional sedative intervention.

Time frame: Intraoperatively to procedure completion.

What these dates mean
Start.
When the first participant enrolled; while estimated, when the study expects to open to recruitment.
Primary completion.
When the last participant was examined or treated to collect data for the study's main question, its primary outcome. A study that stopped early gives the date its data collection stopped.
Completion.
When the last participant's last visit took place, for every outcome, secondary ones and side effects included: the end of the study. Often later than primary completion, sometimes the same day.
Actual and Est.
Actual means the registry says the date has happened. Est. (estimated) is a date the sponsor plans or expects. It can still move, and the sponsor is expected to change it to actual once it has passed.
