An observational study in Stroke, Ideomotor Apraxia and Hemiplegia, sponsored by Izmir City Hospital. Not yet recruiting at 1 site in Turkey (Türkiye). Open to participants aged 18 Years to 90 Years. Per ClinicalTrials.gov, last updated 2026-10-19.
Sponsored by Izmir City Hospital · Observational
The goal of this observational study is to examine whether ideomotor apraxia is associated with motor function, balance, walking performance, and independence in daily activities in adults with hemiplegia after stroke. Ideomotor apraxia is a condition in which a person has difficulty planning and correctly performing learned movements, despite understanding the instructions. It is common after stroke, particularly when the left side of the brain is affected.
The main questions this study aims to answer are:
Participants will attend one assessment session during routine follow-up at the Physical Medicine and Rehabilitation Clinic. They will undergo standardized clinical assessments of apraxia, upper limb motor function, balance, walking performance, and independence in daily activities. No treatment, medication, blood sampling, or imaging will be performed as part of this study.
This single-center, observational, cross-sectional study will investigate the association between ideomotor apraxia and functional outcomes in adults with post-stroke hemiplegia.
Ideomotor apraxia may interfere with the performance of purposeful movements and everyday tasks. Evaluating its relationship with motor impairment and functional performance may help clarify its relevance to rehabilitation assessment after stroke.
Participants will be evaluated during a single session at the Physical Medicine and Rehabilitation Clinic. Ideomotor apraxia will be assessed using the Test for Upper Limb Apraxia (TULIA). Upper limb motor impairment will be evaluated using the upper extremity section of the Fugl-Meyer Assessment. Additional standardized assessments will evaluate balance, walking performance, and independence in daily activities.
The primary analysis will compare Motor Functional Independence Measure (Motor FIM) scores between participants with and without ideomotor apraxia. Secondary analyses will examine associations between TULIA scores and measures of upper limb motor function, balance, and walking performance. Potential confounding factors, including demographic and stroke-related characteristics, will be considered in the analyses. Based on a previously reported effect size (Cohen's d = 1.06), with alpha = 0.05 and 90% power, a total of 54 participants will be enrolled, allowing for 10% data loss.
All study procedures are limited to clinical assessments and collection of relevant clinical information. Participation will not alter routine clinical care.
7,441 studies on the registry are indexed under Stroke; 2,012 are open to participants now.
This study's planned enrollment of 54 is below the median of 160 across 1,729 observational studies indexed under Stroke.

Exclusion Criteria:
Other: Standardized clinical assessment
Other: Standardized clinical assessment
Single-session assessment including the Test for Upper Limb Apraxia (TULIA), Fugl-Meyer Assessment-Upper Extremity, Berg Balance Scale, Functional Ambulation Categories, 10-Meter Walk Test, and Functional Independence Measure. No treatment is administered.
Motor Functional Independence Measure (Motor FIM) Score
The Motor FIM comprises 13 items assessing self-care, sphincter control, transfers, and locomotion. Scores range from 13 to 91, with higher scores indicating greater functional independence. Motor FIM scores will be compared between participants with and without ideomotor apraxia (defined by the Test for Upper Limb Apraxia \[TULIA\] cut-off), and their association with the TULIA total score will be examined.
Time frame: At enrollment, during a single assessment session
Fugl-Meyer Assessment Upper Extremity Motor Score
Upper limb motor impairment will be assessed using the upper extremity motor section of the Fugl-Meyer Assessment. Scores range from 0 to 66, with higher scores indicating better motor function. Its association with the TULIA total score will be examined.
Time frame: At enrollment, during a single assessment session
Functional Ambulation Categories Score
Walking independence will be assessed using the Functional Ambulation Categories, scored from 0 to 5. Higher scores indicate greater walking independence and less need for assistance from another person. Its association with the TULIA total score will be examined.
Time frame: At enrollment, during a single assessment session
Comfortable Walking Speed on the 10-Meter Walk Test
Comfortable walking speed will be assessed using the 10-Meter Walk Test and expressed in meters per second. Higher values indicate faster walking speed. Its association with the TULIA total score will be examined.
Time frame: At enrollment, during a single assessment session
Total Functional Independence Measure Score
Overall functional independence will be assessed using the 18-item Functional Independence Measure, covering motor and cognitive domains. Total scores range from 18 to 126, with higher scores indicating greater independence. Its association with the TULIA total score will be examined.
Time frame: At enrollment, during a single assessment session
Berg Balance Scale Score
Balance will be assessed using the Berg Balance Scale, comprising 14 items scored from 0 to 4. Total scores range from 0 to 56, with higher scores indicating better balance. Its association with the TULIA total score will be examined.
Time frame: At enrollment, during a single assessment session
Test for Upper Limb Apraxia (TULIA) Total Score
Ideomotor apraxia will be assessed through imitation and pantomime of meaningless, intransitive, and transitive gestures. Scores range from 0 to 240, with lower scores indicating more severe apraxia. Participants scoring below the established cut-off will be classified as having ideomotor apraxia.
Time frame: At enrollment, during a single assessment session.
Plan to share: No — Individual participant data will not be shared with other researchers to protect participant privacy and confidentiality. Study findings will be reported in aggregate form through scientific publications and presentations.
