Study summary

The goal of this clinical trial is to evaluate the combination of zanzalintinib (XL092) and tegavivint in advanced hepatocellular carcinoma (HCC) and Wnt pathway mutations who have progressed after at least one prior line of systemic therapy and are naïve (not previously treated with) standard of care tyrosine kinase inhibitors targeting vascular endothelial growth factor signaling.

Read the detailed description

This Phase I/II investigator-sponsored trial will evaluate the combination of zanzalintinib (XL092) and tegavivint in participants with advanced hepatocellular carcinoma (HCC) and Wnt pathway mutations (CTNNB1, AXIN1, CREBBP, APC) who have progressed after at least one prior line of systemic therapy and are VEGFR-TKI naïve. The study employs a two-part design beginning with a Phase I dose escalation using a Bayesian Optimal Interval (BOIN) design with cohorts of 3-6 participants per dose level to determine the safety and tolerability of the combination. The dose escalation will evaluate tegavivint in combination with fixed-dose zanzalintinib. A maximum of 12 evaluable participants will be enrolled in the dose escalation phase to determine the maximum tolerated dose (MTD) and recommended phase 2 dose (RP2D) of tegavivint when combined with zanzalintinib.

What researchers measure

Primary outcomes

  1. Safety/Tolerability as measured by number of subjects with dose limiting toxicity

Safety/Tolerability is measured by the number of participants treated with different dose levels of tegavivint in combination with zanzalintinib who experience dose limiting toxicity adverse events (AE) \>= Grade 3, as defined by Common Terminology Criteria for Adverse Events by CTCAE v5.0.

Time frame: First administration of tegavivint in combination with zanzalintinib up to 24 months

  2. Determine Maximum Tolerated Dose (MTD)

To estimate the MTD and/or to identify the recommended dose of tegavivint for combination zanzalintinib, the variable is the incidence of DLTs in the first cycle of treatment. Estimation of the MTD(s)/recommended dose(s) will be based on a BOIN dose escalation design and dose expansion to estimate the probability of DLT in the first cycle of treatment for participants in the DDS. Following completion of the combination dose escalation, all safety, Dose-determining analysis set (DDS) The DDS includes all participants from the safety set who either completed a minimum exposure requirement or who had a DLT during the first cycle of treatment. The minimum exposure requirement is defined as having received ≥ 80% of the intended tegavivint + zanzalintinib dose(s) during the first cycle.

Time frame: First administration of tegavivint in combination with zanzalintinib to the end of Cycle 1 (each cycle is 28 days).

  3. Recommended phase 2 dose (RP2D) of tegavivint in combination with zanzalintinib

To determine the recommended phase 2 dose (RP2D) of tegavivint in combination with zanzalintinib. The dose determining set will include all participants who either completed a minimum exposure requirement or who had a DLT during the first cycle of treatment. The minimum exposure requirement is defined as having received ≥ 80% of the intended tegavivint + zanzalintinib dose(s) during the first cycle. The RP2D of tegavivint in combination with zanzalintinib will be determined by toxicity assessed by NCI CTCAE v5.0 toxicity criteria.

Time frame: First administration of tegavivint in combination with zanzalintinib to 24 months

Secondary outcomes

  1. Frequency and severity of adverse events

To determine the frequency and severity of adverse events as assessed by NCI CTCAE version 5.

Time frame: Initial treatment until disease progression, unacceptable toxicity, death, or discontinuation from the study treatment for any other reason up to 24 months

  2. Overall Response Rate (ORR)

To determine the overall response rate of combination tegavivint plus zanzalintinib based on Investigator assessment, per RECIST v.1.1. The objective response rate is defined as the proportion of participants with a best overall response (BOR) of complete response (CR) or partial response (PR). ORR and its 95% confidence intervals will be summarized. BOR for each patient is determined from the assessment of response for overall tumor lesions according to the following rules: * CR = at least two determinations of CR at least 1 cycle apart before progression * PR = at least two determinations of PR or better at least 1 cycle apart before progression (and not qualifying for a CR) * SD = stable disease for \> 4 cycles (±14 days) after Cycle 1 Day 1 (and not qualifying for CR or PR) * PD = progression \< 4 cycles after Cycle 1 Day 1 (and not qualifying for CR, PR or SD)

Time frame: Study treatment will continue until disease progression, unacceptable toxicity, death, or discontinuation from the study treatment for any other reason up to 24 months

  3. Disease Control Rate (DCR)

To determine the disease control rate of combination tegavivint plus zanzalintinib based on Investigator assessment, per RECIST v.1.1. The disease control rate is defined as defined as the proportion of participants with BOR of CR or PR or SD.

  4. Duration of response (DOR)

To evaluate efficacy of tegavivint plus zanzalintinib by RECIST 1.1 in participants by DOR. DOR only applies to participants whose BOR is CR or PR.

Time frame: Date of first documented response of CR or PR, whichever is first recorded, and the end date is defined as the date of the first document progression (taking as reference for progressive disease the smallest measurements recorded since to 24 months.

  5. Time to response (TTR)

To evaluate preliminary efficacy of tegavivint plus zanzalintinib by RECIST 1.1 in participants by TTR. TTR is defined as the time from Cycle 1 Day 1 to the first documented response, CR or PR (whichever is first recorded), which must be subsequently confirmed.

Time frame: Cycle 1 Day 1 to the end of Cycle 1 (each cycle is 28 days).

  6. Progression-free survival (PFS)

Progression-free survival (PFS) PFS will be defined as the duration of time from start of the investigational product to the date of first documented disease progression, whichever occurs first.

Time frame: Study treatment will continue until disease progression, unacceptable toxicity, death, or discontinuation from the study treatment for any other reason up to 24 months

  7. Overall survival (OS)

To determine the overall survival rate of response assessed by RECIST guidelines (version 1.1) OS will be defined as the duration of time from start of the investigational product until date of death due to any cause. OS will be collected for all participants who start treatment.

Time frame: Study treatment will continue until disease progression, unacceptable toxicity, death, or discontinuation from the study treatment for any other reason up to 24 months
