Study summary

Neurological outcome is the primary outcome for post-cardiac arrest patients. Hypoxic-ischemic brain injury is a key element in post-cardiac arrest syndrome, and remains the leading cause of mortality and morbidity in post-cardiac arrest patients. In patients with hypoxic-ischemic brain injury, impaired cerebral autoregulation makes cerebral blood flow highly vulnerable to blood pressure fluctuations, which might trigger secondary brain injuries. Although the pressure reactivity index (PRx) calculated via invasive intracranial pressure (ICP) monitoring is considered the gold standard for assessing cerebral autoregulation, its clinical application in post-cardiac arrest patients is limited due to invasive risks. Alternatively, cerebral near-infrared spectroscopy (NIRS) combined with hemodynamic monitoring offers a non-invasive approach to measure the cerebral oxygenation index (COx). However, its consistency with invasive methods and its accuracy in post-cardiac arrest care warrant further validation. The aim of this study is to assess cerebral autoregulation via COx calculated with NIRS and hemodynamic parameters in post-cardiac arrest patients, and to analyze the impact of this index on neurological outcomes. The further goal is to estimate optimal mean arterial pressure (MAPopt) and subsequently establish personalized post-resuscitation blood pressure targets.

ElapsedEstimatedEstimated, not yet reachedSimilar studies: half by the dot; the band is the middle halfHalf of similar studies reached it by the dot; the band is the middle half, from a quarter to three quarters

Start is estimated for Jan 2027; primary completion for Jul 2029, 30 months later.

Of 172 non-industry observational studies under Heart Arrest, half reached primary completion within 39 months of starting (the median), counting those still running; a quarter within 23 and three quarters within 78. This study's planned 30 months is below that.

Counting every sponsor, half of 179 studies reached it within 41 months; a quarter within 23 and three quarters within 78.

How these figures are worked out, and which studies are left out of them.

What these dates mean
Start.
When the first participant enrolled; while estimated, when the study expects to open to recruitment.
Primary completion.
When the last participant was examined or treated to collect data for the study's main question, its primary outcome. A study that stopped early gives the date its data collection stopped.
Completion.
When the last participant's last visit took place, for every outcome, secondary ones and side effects included: the end of the study. Often later than primary completion, sometimes the same day.
Actual and Est.
Actual means the registry says the date has happened. Est. (estimated) is a date the sponsor plans or expects. It can still move, and the sponsor is expected to change it to actual once it has passed.
