  10. Active Hepatitis B or Hepatitis C infections.

  13. Chronic atrial fibrillation with uncontrolled heart rate.
  14. Second primary malignancy that has required therapy in the last 2 years or is not in complete remission. This exclusion criterion does not exclude the following subjects: successfully treated non- metastatic basal cell or squamous cell skin carcinoma, or prostate cancer under control with hormonal therapy
  15. Subjects who have had a venous thromboembolic event (e.g., pulmonary embolism or deep vein thrombosis) requiring anticoagulation and who meet any of the following criteria:

    1. Have been on a stable dose of anticoagulation for \< 1 month (except for acute line insertion induced thrombosis).
    2. Have had a Grade 2, 3, or 4 hemorrhage in the last 30 days
    3. Are experiencing continued symptoms from their venous thromboembolic event (e.g. continued dyspnea or oxygen requirement).
  16. Pregnant or lactating women.
  17. Contraindication to a study treatment/procedure or is anticipated to receive treatment/procedure that may preclude performance of study procedures.
  18. Subjects with Absolute Lymphocyte Count (ALC) ≤0.3 × 109/L or \< 300 cells/mm3 (due to potential challenges with producing CART cells), per investigator judgement

Study arms

  • Experimental
    GPRC5D CART

In the Phase 1a dose-escalation phase, eligible participants for Cohorts 1, 2, 3 and 4 will receive a single dose of either 50× 10˄6, 150 × 10˄6, 300 × 10˄6 or 450 × 10˄6 GPRC5D CART cells, respectively. The Phase 1b expansion part will enroll additional participants at the maximum tolerated dose, as determined during dose escalation.

    Biological: GPRC5D CART

Interventions

  • BiologicalGPRC5D CART

The GPRC5D CART is defines as autologous bone marrow derived T-cells transduced with anti-GPRC5D CAR lentiviral vector encoding the chimeric antigen receptor (CAR) targeted to human GPRC5D. The GPRC5D CART is provided cryopreserved.

What researchers measure

Primary outcomes

  1. Incidence of Treatment-Related Serious Adverse Events and Adverse Events of Special Interest following GPRC5D CART Therapy

Incidence of Treatment-Related Serious Adverse Events and Adverse Events of Special Interest will be assessed

Time frame: 24 months following infusion

  2. Maximum tolerated dose (MTD) of GPRC5D CART cells in Phase 1a.

The maximum tolerated single dose (MTD) of GPRC5D CART cells will be determined during Phase 1a dose-escalation stage.

Time frame: 21 days following infusion

Secondary outcomes

  1. Overall response rate

Overall Response Rate is defined by proportion of subjects with Partial Response or better per IMWG Criteria for MM, and Partial Response or better per Hematologic Response Criteria for AL

Time frame: 24 months following infusion

  2. Complete response rate

Complete Response Rate is defined by proportion of subjects with Complete Response per IMWG Criteria for MM or per Hematologic Response Criteria for AL

Time frame: 24 months following infusion

  3. Progression-Free Survival

Progression-Free Survival is defined as the time from the date of the initial infusion the date of first documented disease progression, or death due to any cause, whichever occurs first

Time frame: 24 months following infusion

  4. Overall Survival

Overall Survival is defined by the date of the initial infusion to the date of the patient's death due to any cause

Time frame: 24 months following infusion

  5. MRD negativity rate

MRD Negativity Rate is defined by proportion of subjects with MRD, defined as the absence of clonal plasma cells on bone marrow aspirate (minimum test sensitivity to detect 1 in 10x5 nucleated cells (10-5 threshold))

    Time frame: At 1, 6, 12, 18, and 24 months

Start is estimated for Nov 2026; primary completion for Dec 2032, 73 months later.

Of 197 non-industry phase 1 interventional studies under Multiple Myeloma, half reached primary completion within 57 months of starting (the median), counting those still running; a quarter within 39 and three quarters within 99. This study's planned 73 months is above that.

Counting every sponsor, half of 402 studies reached it within 50 months; a quarter within 32 and three quarters within 76.

What these dates mean
Start.
When the first participant enrolled; while estimated, when the study expects to open to recruitment.
Primary completion.
When the last participant was examined or treated to collect data for the study's main question, its primary outcome. A study that stopped early gives the date its data collection stopped.
Completion.
When the last participant's last visit took place, for every outcome, secondary ones and side effects included: the end of the study. Often later than primary completion, sometimes the same day.
Actual and Est.
Actual means the registry says the date has happened. Est. (estimated) is a date the sponsor plans or expects. It can still move, and the sponsor is expected to change it to actual once it has passed.
