This is a prospective, single-arm, phase II clinical trial evaluating the efficacy and safety of QLS4131 in patients with relapsed/refractory multiple myeloma (RRMM) with extramedullary disease (EMD).
3,717 studies on the registry are indexed under Multiple Myeloma; 754 are open to participants now.
This study's planned enrollment of 55 is above the median of 41 across 2,967 interventional studies indexed under Multiple Myeloma.

1. Participants who are aware of and voluntarily sign the informed consent form (ICF), aged ≥18 years at the time of signing the ICF; male or female.
2. ECOG performance status of 0-2. 3. Diagnosis of multiple myeloma according to the International Myeloma Working Group (IMWG) criteria, and meeting the following criteria:
Extramedullary disease identified by imaging examination, including the following two categories:
Paraskeletal disease (PSD): lesions formed by extension of bone lesions through the cortical bone into adjacent soft tissue, with at least one lesion measuring ≥5 cm in the longest diameter, or more than 2 paraskeletal lesions measuring ≥2 cm in the longest diameter.
4. Participants who have previously received CAR-T cell therapy or bispecific antibody therapy are allowed. For participants who have previously received BCMA- or GPRC5D-targeted therapy, prior targeted therapy must involve only one of the targets, BCMA or GPRC5D. The interval from CAR-T cell infusion to the first administration of study drug must be ≥3 months, and the interval from the last dose of bispecific antibody to the first administration of study drug must be ≥28 days.
5. Participants without measurable hematologic disease are allowed to enroll. Measurable hematologic disease is defined as meeting any one or more of the following criteria:
<!-- -->
Serum free light chain level ≥10 mg/dL with an abnormal serum free light chain ratio.
6. Adequate organ function. 7. Participants must voluntarily participate and be able to complete the study procedures and follow-up examinations.
Exclusion Criteria:
Participants will receive QLS4131 12 mg by subcutaneous injection. Step-up doses of 0.6 mg and 3 mg will be administered before the first full dose. QLS4131 will be given every 2 weeks during Cycle 1 and every 4 weeks thereafter. Treatment will continue until disease progression, unacceptable toxicity, withdrawal of consent, investigator decision, or completion of 24 cycles.

Start is estimated for Dec 2026; primary completion for Dec 2027, 12 months later.
Of 414 non-industry phase 2 interventional studies under Multiple Myeloma, half reached primary completion within 58 months of starting (the median), counting those still running; a quarter within 40 and three quarters within 95. This study's planned 12 months is below that.
Counting every sponsor, half of 504 studies reached it within 55 months; a quarter within 35 and three quarters within 90.
