An interventional study of Synchronous Telerehabilitation-Based Pilates and Asynchronous Telerehabilitation-Based Pilates in Multiple Sclerosis, sponsored by Yuzuncu Yil University. Recruiting at 1 site in Turkey (Türkiye). Open to participants aged 18 Years to 65 Years. Per ClinicalTrials.gov, last updated 2026-10-26.
Sponsored by Yuzuncu Yil University · Not applicable, Interventional, and Treatment
This study will compare live video-based and prerecorded video-based Pilates programs in 38 adults aged 18-65 years with multiple sclerosis. Participants will be randomly assigned to one of two groups. The live-session group will exercise with a physiotherapist twice a week for eight weeks, with sessions lasting 45-60 minutes. The prerecorded-video group will follow an exercise program matched in intensity and progressed every two weeks, using an exercise tracking form. Trunk muscle performance, balance, thinking and memory, and quality of life will be assessed before and after the program. The study aims to determine whether live supervision provides additional benefits compared with prerecorded exercise videos.
This single-center, assessor-blinded randomized controlled trial will recruit participants receiving routine follow-up at the Multiple Sclerosis Unit of the Department of Neurology, Van Yuzuncu Yil University Faculty of Medicine Hospital.
An independent researcher who is not involved in treatment or outcome assessment will generate the allocation sequence using computer-generated block randomization with a block size of eight. Participants will be assigned to synchronous or asynchronous telerehabilitation. The independent researcher will communicate each participant's allocation to the treating physiotherapist.
The synchronous program will be delivered through live videoconferencing by a physiotherapist certified in Pilates. The asynchronous program will use prerecorded exercise videos matched to the synchronous program in exercise intensity, with progression every two weeks. Participants in the asynchronous group will receive an exercise tracking form to record their participation.
Demographic and clinical information will be collected from participants and medical records. Physical performance assessments will be conducted by a physiotherapist blinded to treatment allocation, and neurological disability will be assessed by a neurologist. Outcome assessments will be performed at baseline and after the eight-week intervention to compare changes within and between groups.

An Expanded Disability Status Scale (EDSS) score between 3.0 and 5.5. Age between 18 and 65 years. Availability of a family member or caregiver to assist with accessing the technological system, if needed.
An active home internet connection for participants assigned to the synchronous telerehabilitation group.
Willingness to participate in the study.
Exclusion Criteria:
Cognitive impairment severe enough to interfere with following instructions. Other diagnosed conditions that would prevent participation, including cardiovascular, visual, or cognitive conditions.
Pregnancy.
Participants will receive an eight-week Pilates exercise program delivered through live videoconferencing by a Pilates-certified physiotherapist. Sessions will be conducted twice weekly, with each session lasting 45-60 minutes.
Behavioral: Synchronous Telerehabilitation-Based Pilates
Participants will receive an eight-week Pilates exercise program using prerecorded exercise videos. Exercise intensity will be matched to the synchronous program, with progression every two weeks. Participants will receive an exercise tracking form to record their participation.
Behavioral: Asynchronous Telerehabilitation-Based Pilates
An eight-week Pilates exercise program delivered by a Pilates-certified physiotherapist through live videoconferencing, twice weekly for 45-60 minutes per session.
An eight-week Pilates exercise program delivered through prerecorded videos, with exercise intensity matched to the synchronous program and progression every two weeks. Participants record their participation on an exercise tracking form.
Change in Trunk Flexor Endurance
Trunk flexor endurance will be assessed by measuring how long participants can maintain a seated position with the trunk at 60 degrees of flexion, hips and knees at 90 degrees, arms crossed over the chest, and feet stabilized. The test ends when the position can no longer be maintained. Duration is recorded in seconds; longer durations indicate better endurance.
Time frame: Baseline and immediately after the 8-week intervention.
Change in Sit-Up Performance
Participants will perform sit-ups from a supine position with knees flexed and feet stabilized by the physiotherapist. The number of successfully completed repetitions will be recorded. Higher repetition counts indicate better performance.
Time frame: Baseline and immediately after the 8-week intervention.
Change in Modified Push-Up Performance
Participants will begin in a prone position with hands at shoulder level and elbows flexed alongside the trunk. They will extend their elbows to lift the head, shoulders, and trunk from the floor. The number of successfully completed repetitions will be recorded. Higher repetition counts indicate better performance.
Time frame: Baseline and immediately after the 8-week intervention.
Change in Activities-Specific Balance Confidence Scale Score
This 16-item self-report scale assesses confidence in maintaining balance during daily activities. Each item is rated from 0 to 100, and the total score is the mean of the item scores. Higher scores indicate greater balance confidence.
Time frame: Baseline and immediately after the 8-week intervention.
Change in Mini-BESTest Score
The Mini Balance Evaluation Systems Test assesses anticipatory postural adjustments, reactive postural control, sensory orientation, and dynamic gait. It includes 14 items scored from 0 to 2, giving a total score of 0-28. Higher scores indicate better balance.
Time frame: Baseline and immediately after the 8-week intervention.
Change in Symbol Digit Modalities Test Score
The Symbol Digit Modalities Test, administered as part of the BICAMS battery, assesses cognitive processing speed. The number of correct symbol-digit substitutions completed within 90 seconds is recorded. Higher scores indicate better performance.
Time frame: Baseline and immediately after the 8-week intervention.
Change in California Verbal Learning Test Score
The California Verbal Learning Test, administered as part of the BICAMS battery, assesses verbal learning and memory. Performance is measured by the total number of words correctly recalled across the initial five learning trials. Higher scores indicate better verbal learning.
Time frame: Baseline and immediately after the 8-week intervention.
Change in Brief Visuospatial Memory Test-Revised Score
The Brief Visuospatial Memory Test-Revised, administered as part of the BICAMS battery, assesses visuospatial learning and memory. Performance is measured by the total recall score across three learning trials. Higher scores indicate better visuospatial learning.
Time frame: Baseline and immediately after the 8-week intervention.
Change in Preference-Based Multiple Sclerosis Index Score
This patient-reported measure assesses health-related quality of life across walking, fatigue, mood, concentration, and roles and responsibilities. Responses are combined into a preference-based index score. Higher scores indicate better health-related quality of life.
Time frame: Baseline and immediately after the 8-week intervention.
