Study summary

ATTN201 is a non-invasive focused ultrasound neurostimulation device that delivers offline MRI-guided non-invasive focused ultrasound to target brain areas. A cranial MRI is obtained before starting treatment while subjects wear a MR-compatible fiducial laced version of the wearable to co-register the cranial coordinate system with transducer coordinates. Simulations will define ultrasound focusing instructions for each patient to specifically target their brain area. For 100+ participants, we will also continuously monitor multiple interstitial pain biomarkers over the course of the intervention.

Read the detailed description

This is an open-label, single-arm study evaluating the safety, feasibility, and efficacy of delivering low intensity focused ultrasound stimulation of the centromedian nucleus of the thalamus (CMN) using the ATTN201 device for the treatment of chronic pain, with integrated multimodal biomarker monitoring.

Following informed consent and a screening (intake) visit, which may be completed either on a separate day before the first on-site visit or on the day of Visit 1 prior to any study procedures, participants will come in for Visit 1 (Day 0) for baseline measurements, safety assessments, MRI, and fitting of monitoring devices (Adaptyx Continuous Biomonitor, EEG, and smart watch; set up of all devices will be done after the MRI). Participants will receive 3 FUS treatments targeting the CMN on Days 3, 5-6, and 8-9; the two days immediately after the MRI/baseline visit (Days 1-2) are non-stimulation monitoring days that allow completion of offline MRI-guided targeting before the first treatment. Participants who do not achieve at least a 30% reduction in BPI-SF Pain Severity at Day 14 (non-responders) may be offered an optional extension of 3 additional sessions targeting the ventroposterolateral nucleus (VPL).

During the 14-day monitoring period (Day 0-14), participants will use multiple monitoring devices with varying durations:

  • Wear the Adaptyx Continuous Biomonitor for interstitial biomarker measurement (Day 0 through Day 8-9; returned at Visit 4)
  • Wear the wireless EEG system for continuous ambulatory EEG (Day 0 through Day 8-9; returned with Adaptyx Continuous Biomonitor at Visit 4)
  • Wear a smart watch for heart rate variability, sleep, and actigraphy data (Day 0 through Day 14; returned at Day 14)
  • Complete Visual Analog Scales (VAS) 3x daily for sleepiness, mood, depression, and arousal, and Consensus Sleep Diary each morning
  • Complete the BPI-SF Pain Severity subscale 3 times daily (morning, afternoon, evening) via the Attune mobile application during the 14-day monitoring period (Day 0-14), alongside the VAS battery, with additional self-initiated entries available anytime. Formal one-time administration is conducted at each follow-up visit (Months 1, 2, 3, and 6). The same anytime channel supports on-demand VAS, free-text notes, voice memos, and AE reports - see Section 4.4.2 and Section 7.1.2.2.11.
  • Complete a brief (\~30-second) voice recording each day in response to the prompt "How was your day?" during the 14-day monitoring period.
  • Have saliva collected via salivette and/or blood collected from them (capillary blood) via a capillary blood collection device, such as a Tasso or RedDrop or venous blood collected via venipuncture.
  • Submit on-demand VAS ratings, free-text notes, voice memos, or adverse-event reports through the Attune mobile application at any time during the study (inside or outside the monitoring period) whenever a change in pain, mood, sleep, or anything else of interest is noticed.
  • Complete BPI-SF Pain Interference, PHQ-9, ISI, GAD-7, DASS-8, Brief Resilience Scale, and Flourishing Scale at baseline (Day 0), Day 8, and Day 14 Follow-up assessments at Months 1, 2, 3, and 6 post-baseline will capture the full clinical scale battery: BPI-SF (Severity and Interference), PHQ-9, ISI, GAD-7, DASS-8, Brief Resilience Scale, and Flourishing Scale.

Participants may report adverse events at any time through the Attune mobile application or through direct communication with the PI/study staff.

What researchers measure

Primary outcomes

  1. Change in Brief Pain Inventory - Short Form (BPI-SF) Pain Severity Subscale Score

The Brief Pain Inventory - Short Form (BPI-SF) Pain Severity subscale is a 4-item self-report measure (worst pain, least pain, average pain, and current pain), each item rated on a 0 to 10 numeric rating scale anchored at 0 = "No pain" and 10 = "Pain as bad as you can imagine." The subscale score is the arithmetic mean of the 4 items and ranges from a minimum of 0 to a maximum of 10. Higher scores indicate greater pain severity, i.e., a worse outcome. Change is calculated as the Day 14 score minus the baseline score; a negative change indicates improvement.

Time frame: Baseline (Day 0) and Day 14

Secondary outcomes

The subscale score is the arithmetic mean of the 4 items and ranges from a minimum of 0 to a maximum of 10. Higher scores indicate greater pain severity, i.e., a worse outcome. This measure assesses durability of the treatment effect at Month 3, the key secondary timepoint.

Each item is rated on a 0 to 10 scale anchored at 0 = "Does not interfere" and 10 = "Completely interferes." The subscale score is the arithmetic mean of the 7 items and ranges from a minimum of 0 to a maximum of 10. Higher scores indicate greater interference from pain, i.e., a worse outcome.
