Study summary

This clinical trial will test individual wellness coaching for 44 nurses who provide cancer care. Wellness means balance across physical, emotional, social, and other areas of life.

The main questions are:

Does wellness coaching improve nurses' quality of life at work?

Does wellness coaching improve nurses' wellness?

Nurses will be grouped according to their work area: inpatient wards, intensive care units, or outpatient clinics. Within each work area, researchers will assign nurses by chance to one of two groups. Each nurse will have an equal chance of joining either group.

One group will receive individual wellness coaching for eight weeks. The control group will continue usual work routines without receiving wellness coaching or educational materials from the research team during the assessment period. An informational brochure about wellness coaching will be offered after completion of all scheduled outcome assessments, including the assessment three months after the intervention period. Researchers will compare changes in quality of life and wellness between the groups.

Participants will:

Attend individual coaching sessions in person if assigned to the coaching group. Sessions will take place once a week for eight weeks. Each session will last about 45 to 50 minutes.

Answer questionnaires before the intervention period and immediately after it.

Answer questionnaires again one and three months after the intervention period.

Coaching will focus on exploring personal needs and setting goals. Topics will include strategies for looking after their health, managing stress, and balancing work and personal life.

Read the detailed description

This randomized controlled trial will include 44 oncology nurses assigned to two parallel groups. Participants will be stratified by work area: inpatient ward, intensive care unit, or outpatient clinic. Within each stratum, participants will be individually randomized in a 1:1 ratio to receive individual wellness coaching for eight weeks or to join the control group.

An independent person who is not involved in recruitment, coaching, or outcome assessment will use a computer to generate separate randomization sequences for each stratum, using permuted blocks of randomly varying sizes. Sequentially numbered, opaque, sealed envelopes will be prepared for each stratum. Recruitment staff will not have access to the allocation sequences. After eligibility confirmation, written informed consent, and baseline assessment, an independent allocator will open the next envelope in the relevant stratum in numerical order.

The coaching group will receive eight individual coaching sessions in person, delivered once a week for eight weeks. Each session will last approximately 45 to 50 minutes. Coaching will address eight wellness dimensions, with session content, questions, and goals tailored to individual needs. Baseline Wellness Wheel assessments will guide the identification of personal priorities.

The control group will continue usual work routines without receiving wellness coaching or educational materials from the research team during the assessment period. An informational brochure about wellness coaching will be offered after completion of all scheduled outcome assessments, including the assessment three months after the intervention period.

Outcomes will be assessed using the Turkish Professional Quality of Life Scale, Version IV, and the Wellness Wheel form developed by the researchers. Assessments will take place at baseline, immediately after the intervention period of eight weeks, and one and three months after intervention completion.

Participants and the researchers delivering coaching will be aware of group assignments. Outcomes will be reported by the participants. The data analyst will receive a dataset with groups coded as A and B and will remain unaware of which code represents each group until the planned analyses have been completed.

Interventions

  • BehavioralIndividual Wellness Coaching

An eight-week individual wellness coaching program delivered face-to-face through weekly sessions lasting approximately 45-50 minutes. The program addresses physical, emotional, social, mental, occupational, financial, spiritual, and environmental wellness. Baseline Wellness Wheel self-assessments will guide individual priorities. Sessions will include reviewing goals, exploring personal resources, practicing relevant self-care and stress management strategies, and setting a small, measurable goal for the following week.

What researchers measure

Primary outcomes

  1. Change From Baseline in Professional Quality of Life: Compassion Satisfaction Subscale Score

Compassion satisfaction will be assessed using the Turkish Professional Quality of Life Scale, Version IV (ProQOL-IV). The Compassion Satisfaction subscale contains 10 items rated from 0 to 5. Item scores are summed to obtain a subscale score ranging from 0 to 50. Higher scores indicate greater compassion satisfaction. Change will be calculated by subtracting the baseline score from the score at each subsequent assessment.

Time frame: Baseline, immediately after the 8-week intervention, and 1 and 3 months after intervention completion.

  2. Change From Baseline in Professional Quality of Life: Burnout Subscale Score

Burnout will be assessed using the Turkish Professional Quality of Life Scale, Version IV (ProQOL-IV). The Burnout subscale contains 10 items rated from 0 to 5. After reverse scoring the designated items according to the instrument's scoring instructions, item scores are summed to obtain a subscale score ranging from 0 to 50. Higher scores indicate greater burnout. Change will be calculated by subtracting the baseline score from the score at each subsequent assessment.

Time frame: Baseline, immediately after the 8-week intervention, and 1 and 3 months after intervention completion.

  3. Change From Baseline in Professional Quality of Life: Compassion Fatigue/Secondary Traumatic Stress Subscale Score

Compassion fatigue/secondary traumatic stress will be assessed using the Turkish Professional Quality of Life Scale, Version IV (ProQOL-IV). This subscale contains 10 items rated from 0 to 5. Item scores are summed to obtain a subscale score ranging from 0 to 50. Higher scores indicate greater compassion fatigue/secondary traumatic stress. Change will be calculated by subtracting the baseline score from the score at each subsequent assessment.

Time frame: Baseline, immediately after the 8-week intervention, and 1 and 3 months after intervention completion.

  4. Change From Baseline in Wellness Wheel Total Score

Perceived wellness will be assessed using a researcher-developed Wellness Wheel self-assessment form covering eight domains: physical, emotional, social, intellectual, occupational, financial, spiritual, and environmental wellness. Participants rate each domain from 0 (not well at all/no balance) to 10 (excellent balance and satisfaction). Domain scores are summed to obtain a total score ranging from 0 to 80. Higher scores indicate better perceived wellness. Change will be calculated by subtracting the baseline score from the score at each subsequent assessment.

Time frame: Baseline, immediately after the 8-week intervention, and 1 and 3 months after intervention completion.
