Study summary

The goal of this observational study is to document clinical outcomes after primary hip arthroscopy in patients with femoroacetabular impingement, describe the pathology zone distribution and detect whether the pathology site impact the outcomes. The main questions it aims to answer are:

  • Does hip-related quality of life improve from before surgery to 12 months after surgery?
  • Where inside the hip joint (front, mixed, or back) does cartilage and labral damage most commonly occur in patients undergoing arthroscopy for FAI?
  • Do pain and quality-of-life outcomes at 12 months differ between patients based on the hip damage region?

Participants will

  • Complete questionnaires about hip pain and function (iHOT-12, a pain scale, and a quality-of-life questionnaire) before surgery
  • Undergo standard hip arthroscopy, during which the surgeon will record the exact location and grade of any joint damage found
  • Complete the same questionnaires again 12 months after surgery
Read the detailed description

Femoroacetabular impingement (FAI) - abnormal contact between the proximal femur and acetabular rim during hip motion - is an established cause of hip pain and early osteoarthritis in active adults. FAI classically presents with anterior groin pain, found in 81% of patients in a large arthroscopic series. In these patients, cam or pincer morphology produces chondrolabral injury predominantly at the anterosuperior acetabular rim, between the 12 and 3 o'clock positions. Posterior hip or buttock pain, however, occurs in up to 20% of patients with early intra-articular hip disease, and atypical pain locations account for 29.6% of consecutive FAI patients undergoing arthroscopy.

Posterolateral FAI may result from a cam deformity at the 9-11 o'clock arc of the femoral head-neck junction, from posterior acetabular overcoverage due to a prominent posterior wall, or from abnormal femoral version shifting the impingement zone posteriorly - each producing posterior chondrolabral injury distinct from the anterosuperior pattern of classical anterior cam FAI.

The evidence base for posterolateral FAI as a defined entity remains sparse. Previous cohorts were retrospective and used pain location as their sole stratification criterion, without intraoperative mapping of the pathological zone. No prospective cohort has systematically documented the intraoperative clock-face distribution of chondrolabral pathology in consecutive FAI patients or compared morphological and patient-reported outcomes across pathology-zone groups.

This study aims to document the clinical course of consecutive FAI patients undergoing hip arthroscopy, describe intraoperative distribution of chondrolabral pathology of the cohort by clock-face position with its respective clinical features; to identify the preoperative morphological correlates of each pathology-zone group; and to compare outcomes at 12 months between groups as pre-specified exploratory secondary analyses.

What researchers measure

Primary outcomes

  1. Change in iHOT-12 from baseline to 12 months for the whole cohort

Participants will complete a questionnaire for the iHOT-12

Time frame: measured prior to and 12 months after surgery

Secondary outcomes

  1. Frequency distribution of chondrolabral pathology by clock-face position; proportion of patients in each zone group (A, B, C)

Intraoperative Assessment will be done via a structured assessment form and the distribution of chondrolabral pathology by clock-face positionfor each patient is documented. Then each patient is classified intraoperatively as: Group A (anterior/anterolateral pathology, 12 to 3 o'clock arc), Group B (mixed anterior and posterolateral pathology), or Group C (isolated posterolateral pathology, 9 to 11 o'clock arc).

Time frame: Done at hip arthroscopy surgery

  2. VAS score change

VAS score for pain to be filled by the patient

Time frame: prior to and at 12 months after surgery

  3. Change in EQ-5D-5L from baseline to 12 months

EQ-5D-5L questionnaire to be filled by the patient

Time frame: prior to and at 12 months after surgery

  4. Tönnis grade

AP pelvis and lateral hip radiographs are obtained and assessed for differents variables as: Tönnis grade.

Time frame: prior to hip arthroscopy

  5. LCEA

AP pelvis and lateral hip radiographs are obtained and assessed for differents variables as: Lateral Centre Edge Angle

Time frame: prior to hip arthroscopy

  6. PWI

AP pelvis and lateral hip radiographs are obtained and assessed for differents variables as: Posterior Wall Index

Time frame: prior to hip arthroscopy

  7. Alpha angle

AP pelvis and lateral hip radiographs are obtained and assessed for differents variables as: Alpha angle

Time frame: prior to hip arthroscopy

  8. HNOR

AP pelvis and lateral hip radiographs are obtained and assessed for differents variables as: Head Neck Offset Ratio.

Time frame: prior to hip arthroscopy

  9. Acetabular version

    Assessed via rotational profile images

Time frame: prior to hip arthroscopy

Other outcomes
